Patient Information

First Visit

Your initial visit is a crucial step in the treatment planning process.  During this visit, we will get to know you by reviewing your health history, performing an examination, explaining your diagnosis, and creating a treatment plan.   Your consultation visit will conclude with a brief visit with one of our financial coordinators.  They will go over the proposed treatment plan and the associated fees and find a convenient time to schedule your surgery.

Occasionally, surgery can be performed the same day as the consultation, should you choose to and are prepared.  (You must have an escort to drive you home).  

Please assist us by providing the following information at the time of your first visit:

• Referral form or letter from your referring dentist/doctor
• X-rays that have been taken by your referring dentist
• A list of all medications you are presently taking
• Picture ID
• Your dental and/or medical insurance cards

Important:  All patients under 18 years of age must be accompanied by a parent or legal guardian for consultation and surgery.

Scheduling

W.I.S.D.O.M.  Oral and Maxillofacial Surgery is open Monday - Friday from 8:00 am until 5:00 pm. Your appointment will be scheduled as quickly as possible, and special considerations will be given to emergency situations for same-day consultations.

Our goal is to provide a prompt service, however, there may be times that certain surgical procedures may take longer than the allocated time. Emergency cases requiring immediate attention may cause delays. We appreciate your understanding and patience.

Financial Information

For your convenience, we accept Visa, MasterCard, and CareCredit.

Our goal is to provide the finest care at a reasonable cost. Unless previous arrangements have been made, payment is due at the services are rendered.

You are responsible for all fees charged by this W.I.S.D.O.M. Oral Surgery regardless of your insurance coverage.

You will receive a monthly statement, and insurance companies typically respond within 4-6 weeks. Please contact us immediately if your statement does not reflect your insurance payment. You are respobsible for any balance after your insurance has paid. If you are interested in a monthly payment plan, we can make arrangements prior to your procedure.

Dr. Khayat,

Thank you for a successful surgery during my Christmas break. Your flexibility was greatly appreciated as my wisdom teeth only started to hurt less than a month before my break. Being able to get my teeth taken care of over break saved me the trouble of dealing with sore teeth until summer. I also want to compliment your professional competence. The surgery went so well and was complication free that I did not need the pain killers (though they were within arms reach at all times in case of need). By following your instructions for post-op care I was able to get back to my regular diet in a few days, and minimized swelling so well that it was barely noticeable (though my mom swore she could see it everyday). The get well card was unexpected and thoroughly appreciated. Once again thank you for the successful surgery.
